I am unclear on what you are asking.  Surely you must be aware by now that 
Joop's Polling Booth is already primed with ballot questions that, 
essentially, include the three you listed, somwhat restated.  And he is about 
to launch his poll (on his own initiative).  Both he and you advocate very 
aggressive voting schedules that, essentially, ignore the sick, those who are 
away on business travel or vacation, or who, like myself, simply have a hard 
time keeping current with our e-mail, as Judyth has pointed out.

As I have repeatedly stated, I volunteered to oversee a poll, not conduct one 
(both for ethical and very practical reasons).
